The project OptiMed trial: Effectiveness of a medication management programme for polymedicated home-dwelling older adults: development phase of a pragmatic randomised controlled trial will support the development phase of a pragmatic randomised controlled trial to assess the effectiveness of an innovative, coordinated, comprehensive, community-based model for supporting medication management and preventing medication-related problems among polymedicated, home-dwelling older adults with multiple chronic conditions.
